Should be avoided during pregnancy and while breastfeeding, and by anyone with existing liver disease. There are case reports connecting Gotu Kola to liver injury, and though a direct causal link isn't fully settled, caution is warranted — avoid combining with other substances that stress the liver, including alcohol. Not recommended for continuous, uninterrupted long-term use; traditional practice limits a course to a few weeks followed by a break. May interact with sedatives, diabetes medications, cholesterol-lowering drugs, and blood thinners. Stop use at least two weeks before any scheduled surgery.
