Internal use of neem OIL specifically carries real, documented danger: severe poisoning and infant deaths have been reported from oral neem oil, and it should never be given to infants or young children by mouth under any circumstance. Neem is a genuine pregnancy contraindication — it has been associated with miscarriage when taken by mouth and should be avoided entirely during pregnancy; safety during breastfeeding is also not established. It has documented anti-fertility effects and may harm sperm, so those trying to conceive (of any sex) should avoid it. Diluted neem oil applied to intact skin is considered reasonably safe for short-term use, but always patch-test first and dilute in a carrier oil, since neem contains many active compounds. May lower blood sugar (caution with diabetes medication) and may increase immune system activity, so use caution or avoid if you have an autoimmune condition. Stop use at least 2 weeks before surgery.
